Dr. Kezirian sees patients at offices in Santa Monica and Beverly Hills but can also schedule video consultations for patients who would like an appointment before or instead of traveling to Southern California.
Scheduling an appointment
For an appointment with Dr. Kezirian, please contact the UCLA Department of Head and Neck Surgery call center directly at 424-259-6559 (Santa Monica) or 310-205-7310 (Beverly Hills) . For patients with medical records from outside the UCLA system, please either have them sent at least 3 days in advance of the appointment or bring them with you to the appointment. These records include completed new patient forms listed below (if you have not completed them online) and reports from all previous sleep studies. It takes some time to fill out the forms, and doing so in advance is the best thing you can do to be seen on schedule. Medical records can be sent via e-mail ([email protected]), fax (310-267-5636), or regular mail (Medical Records, UCLA Medical Center, 10833 Le Conte Avenue, CHA BH239, Los Angeles, CA 90095). Bringing these records with you to the appointment will ensure that they are available for the appointment without being lost (or having your team’s office forget to send them). You can obtain records (such as sleep study reports) directly from the physicians or centers that performed them. After your appointment, Dr. Kezirian will send the note from your visit to your doctors.

Please bring the following to your appointment:
- New Patient Forms. Please complete these forms in advance of your appointment. The forms may require asking bed partners or family members about your sleep patterns. In addition to bringing them to your appointment, you may also send completed forms by e-mail to [email protected].
- Copies of full reports (not just 1-2 page summary, if possible) of all previous sleep studies. It is not necessary that you have had sleep studies prior to your appointment, but please bring the reports if you have had the studies done.
- Copies of operative reports from all previous surgeries related to the head and neck.

Video consultations (using a HIPAA-compliant version of Zoom) include review of the completed New Patient Forms, your previous sleep studies, and results from any previous surgeries or other treatments. Postoperative Follow Up Appointments
For patients who have undergone surgery with Dr. Kezirian and are returning after their postoperative sleep study, here is the Sleep Surgery Postoperative Form to complete and bring with you to the appointment. Patients who are seeing Dr. Kezirian for the first time do not need to complete this form.
Traveling to see Dr. Kezirian
For patients who will be traveling to see Dr. Kezirian from outside the Los Angeles area, including from other countries, there are two options. If you would like to schedule an online video consultation prior to traveling for an in-person visit, please contact Dr. Kezirian. If would like to schedule an in-person visit, please contact the office visit scheduling team. There is a Concierge Service (424-259-9915) that can provide assistance with recommendations regarding hotels, restaurants, stores, transportation, religous services, etc.
